Job description
Heartlite Hospice - Scottsboro, part of the LHC Group and Optum family, seeks a Registered Nurse Case Manager to oversee hospice plans of care. You will perform initial nursing evaluations within 48 hours of referral and coordinate care with the IDG to promote comfort and overall well-being.
