Job description
Holisticare Hospice is seeking a Per Diem Hospice RN Case Manager (RNCM) to support patients and families with compassionate end-of-life care in Ocean County. You will collaborate with our interdisciplinary team to coordinate services, deliver hands-on nursing care, and advocate for patient comfort and dignity.
Qualified RNs with at least 1 year of medical-surgical experience, EMR proficiency, CPR certification, and a valid driver’s license are encouraged to apply.
