Job description
Carilion Clinic in Virginia's Franklin County region is seeking an RN Case Manager, Hospice, to provide individualized end-of-life care and coordinate interdisciplinary planning.
You will assess patients across ages, administer treatments, and educate families, while ensuring CMS and agency policies are followed; a sign-on bonus is eligible.
Applicants must be a graduate of an accredited RN program and licensed in Virginia, with a valid driver's license for home visits.
