Job description
Heart of Hospice, part of the LHC Group within the Optum family, is hiring a Registered Nurse Case Manager to lead the plan of care for hospice patients and coordinate interdisciplinary care.
You will evaluate patient needs, develop treatment plans rooted in nursing theory, and collaborate with families, physicians and the IDG to promote comfort and quality of life in home settings. CPR certified and RN licensed in Louisiana preferred.
