Job description
Optum, a part of UnitedHealth Group, is hiring a Registered Nurse Case Manager for hospice care. You will assess patients within 48 hours of referral, develop and coordinate the plan of care, and maintain detailed clinical notes.
You will collaborate with families, physicians, and the IDG to ensure high-quality, compassionate care in a home setting. The role requires an unrestricted RN license, a valid driver's license, and the ability to work in diverse home environments.
