Job description
Community Healthcare of Texas is seeking an RN Case Manager for a full-time role in Fort Worth, TX. You will meet with patients and families to explain hospice services, coordinate admissions, and manage ongoing care plans.
Responsibilities include assessing patient needs, developing care plans, administering treatments, and coordinating with physicians and the interdisciplinary team. A $5,000 sign-on bonus is offered and on-call rotation may be required.
