Job description
Luminary Hospice is seeking a Registered Nurse Case Manager to plan, organize and direct hospice care in home and community settings. You will assess patients, develop and adjust care plans, coordinate with physicians and the interdisciplinary team, and educate families for comfort and quality of life.
The role requires current state nursing licensure, CPR, and 1-2 years hospice or acute care experience. A $5,000 sign-on bonus is offered with details reviewed during hiring.
