Job description
BJC Home Care in the St. Louis area is seeking a Hospice - Registered Nurse Case Manager to coordinate patient care in home settings. Monday to Friday, 8:00am–4:30pm with weekend/on-call rotation.
You will evaluate patient needs, develop care plans with a multidisciplinary team, and ensure timely communication across the continuum, including EMR updates. The role supports professional growth via the BJC RN Career Ladder and may offer relocation for eligible candidates.
