Job description
Chapters Health System is seeking a Registered Nurse (RN) Case Manager in Florida to assess patient needs, coordinate the Plan of Care with the Interdisciplinary Team, and deliver clinical, palliative, and supportive care to patients in their home environment.
This role requires an active Florida RN license, at least 1 year hospice/hospital experience, EMR/EHR proficiency, and a valid driver’s license with reliable transportation.
