Job description
Deercreekseniorliving in Milwaukee County is seeking a Hospice RN Case Manager to provide compassionate, coordinated care for patients under our hospice program. You will handle a caseload of 15–18 patients, performing 5–6 visits per day, Monday through Friday, 40 hours weekly, with no routine on-call or weekend requirements.
Experience in hospice is not required. A company vehicle is provided, and the role offers a $10,000 sign-on bonus.
