Job description
We are hiring a full-time RN Hospice Case Manager to join the Athena Home Health & Hospice team in the Torrington area!
nThis position is on-the-road covering Litchfield County and western Hartford County. Salaried role with weekly pay up to $49 an hour, depending on experience. Shift is Monday-Friday 8a-4:30p. Position is vacant due to relocation.
nAs an RN Hospice Case Manager, you plan, organize, and direct hospice care. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
nExperience & Education:
nnnGraduate of an accredited school of nursing.
n
nnOne (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care.
n
nnActive and unencumbered RN license in Connecticut.
n
nnCPR certification.
n
nnBachelor’s degree, with one (1) year of hospice care experience preferred.
n
n nDuties & Responsibilities:
nnnResponsible for supervising hospice aides.
n
nnCompletes an initial, comprehensive, and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
n
nnProvides professional nursing care by utilizing all elements of the nursing process.
n
nnAssesses and evaluates patient’s status by:
n
nnWriting and initiating plan of care
n
nnRegularly re-evaluating patient and family/caregiver needs
n
nnParticipating in revising the plan of care as necessary
n
nnInitiates the plan of care and makes necessary revisions as patient status and needs change.
n
nnUses health assessment data to determine nursing diagnosis.
n
nnDevelops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
n
nnInitiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
n
nnCounsels the patient and family in meeting nursing and related needs.
n
nnProvides health care instructions to the patient as appropriate per assessment and plan.
n
nnAssists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
n
nnActs as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
