Job description
Optimal Care - Evergreen in Michigan seeks a Hospice RN Case Manager to deliver skilled nursing and compassionate support to patients and families at home or in facilities. You will coordinate comprehensive care for your caseload, building trust with patients, families, physicians, and the interdisciplinary team.
This role requires a registered nurse, valid state license, and at least one year of clinical experience.
