Job description
Community Hospice is seeking a Hospice Registered Nurse to join our Nursing Home Care Team in the Capital Region (Albany area). This full-time day shift role supports patients and families in local nursing homes, with no weekends and no on-call.
You will manage hospice patients, coordinate with nursing home staff, and provide symptom management and emotional support. A NY RN license, ADN or BSN, and LTC experience are required; hospice experience is a plus.
