Job description
Trinity Health At Home Hospice is seeking a dedicated RN Case Manager to provide professional nursing care to hospice patients, coordinating plans of care, and supporting families. This role requires an active Michigan RN license, 1+ year acute care experience, and strong communication skills. A $15,000 sign-on bonus is offered along with day-one benefits.
Schedule is Friday through Sunday, 8:00 a.m. to 9:00 p.m., with 36 hours per week and paid for 40.
