Registered Nurse (RN) Case Manager – Hospice

WHAT YOU'LL DO:
• Coordinate and manage hospice care for your assigned patients • Perform comprehensive patient assessments and develop individualized care plans • Collaborate with physicians and the interdisciplinary hospice team • Provide compassionate education, guidance, and support to patients and families • Maintain accurate and timely documentation in accordance with hospice regulations • Build meaningful relationships with patients and families during an important time in their lives
WHAT WE'RE LOOKING FOR:
Active Pennsylvania RN license Hospice or home health experience preferred Excellent communication and organizational skills Compassionate, patient-centered approach Valid driver's license and reliable transportation
