Job description
Trinity Health Mid-Atlantic's Community Hospice in the Columbia/Greene County area is seeking a full-time RN Clinical Lead to support patient care planning and coordination. The role runs Monday through Friday, 8:00 am to 4:30 pm, and involves delivering end-of-life care in clients' homes with a collaborative, multidisciplinary team.
Training and orientation are provided, with a strong emphasis on teamwork and safety.
