Job description
Gentiva is seeking an experienced Hospice Clinical Liaison to streamline intake, coordinate admissions, and educate patients and families about hospice services. You will build relationships with hospitals, discharge planners, physicians, and referral sources while ensuring compliance with legal and payer requirements.
The role requires nursing licensure with 3–5 years in hospice and a proven ability to educate diverse families with compassion.
