Job description
Commonheart in Fond du Lac, WI is seeking compassionate RN or LPN to join our hospice team. You will provide skilled nursing, collaborate with an interdisciplinary team, and support patients and families with education and advocacy. A valid license and driver's license are required, with travel within a one-hour radius and a company car for eligible roles.
The schedule is 0.8 FTE (32 hrs) or 1.0 FTE (40 hrs) per week, with on-call coverage and competitive hourly pay based on experience.
