Job description
Three Oaks Hospice in Texas is seeking a dedicated registered nurse to administer skilled nursing care for patients in their homes. You will coordinate care with the interdisciplinary team, patients, families, and referring agencies, and oversee care coordination.
Responsibilities include comprehensive assessments, implementing the plan of care, educating families, providing on-call support, and maintaining accurate progress notes.
