Job description
Elysian Hospice in Dallas is seeking an experienced Registered Nurse (RN) to provide skilled hospice care in patients' homes and coordinate with the Interdisciplinary Team (IDT). The RN will perform comprehensive assessments, develop and manage hospice plans of care, and ensure quality patient care and regulatory compliance.
This role requires strong clinical assessment, care coordination, and independent work in a hospice field setting while collaborating with the interdisciplinary care team.
