Job description
Three Oaks Hospice is seeking a qualified registered nurse to administer skilled nursing care for clients in their home. You will coordinate care with the interdisciplinary team, patients, families, and referral agencies, and assume responsibility for care coordination.
Essential duties include conducting initial and ongoing assessments, implementing the plan of care, educating patients and families about end‑of‑life care, and supporting other staff.
