Job description
Brighton Hospice in Avon, CT is seeking a Registered Nurse to join our interdisciplinary team. You will provide patient-centered care by visiting patients in homes, SNFs, and assisted living facilities, and contribute to psychosocial assessments and care planning as part of a case management, admissions, or resource role.
