Job description
Hartford HealthCare at Home is seeking a registered nurse to provide skilled nursing care in clients' homes and community settings. You will assess needs, develop and monitor plans of care, and coordinate with physicians and the IDT to deliver high-quality, cost-effective care.
Teaching families and supporting bereavement needs are part of the role. Applicants should have an Associate degree in nursing and at least one year of nursing experience; home care or hospice experience is preferred.
