Job description
Elysian Hospice in Houston, TX is seeking an experienced Hospice Registered Nurse responsible for administering nursing care to patients in their homes or residences, coordinating care with the interdisciplinary team, patient, family, and referring agencies, and ensuring care coordination.
The RN will perform initial and ongoing assessments, educate families, and develop plans of care while adhering to professional standards.
