About the Role:
Solara Hospice LLC is looking for a compassionate and dedicated Hospice RN to join our team in Dedham, MA. In this meaningful role, you'll provide expert clinical care and emotional support to patients and families during life's most important moments. Responsibilities:
Conduct comprehensive patient assessments and develop individualized hospice care plans
Provide skilled nursing care and symptom management for end-of-life patients in home or facility settings
Educate and support patients and families on disease progression, comfort measures, and hospice philosophy
Collaborate with an interdisciplinary team including physicians, social workers, chaplains, and aides
Administer and manage medications, including pain and palliative care protocols
Document patient care accurately and in compliance with Medicare/Medicaid hospice regulations
Provide on-call support and respond to urgent patient and family needs as needed
Requirements:
Active Registered Nurse (RN) license in the state of Massachusetts
Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
Strong knowledge of pain management, symptom control, and end-of-life care
Excellent communication and interpersonal skills with patients, families, and care teams
Ability to work independently and manage a patient caseload in a field-based setting
Valid driver's license and reliable transportation for home visits
CPR certification and up-to-date clinical credentials required
