VitalCaring – Deliver Care That Brings Comfort and Dignity. The Hospice RN role is field-based in Hammond, LA, serving North Shore patients. You will lead the plan of care, manage symptoms, and provide personalized, comfort-focused care aligned with patient goals.

As the clinical leader for patients and families, you will coordinate with an interdisciplinary team, document precisely, participate in care planning meetings, and advocate for compassionate end-of-life care in homes and facilities.
