Job description
Commonheart in Wisconsin is seeking a compassionate RN or LPN for our hospice team to deliver end‑of‑life care to patients and families with empathy.
You'll collaborate with an interdisciplinary team, implement individualized care plans, educate families, and document care while communicating changes in condition.
Travel within a one‑hour radius is required; a company car is provided for 0.6 FTE+. Schedule is Mon–Fri, 32–40 hours, with on‑call coverage and competitive hourly rates.
