Job description
Housecall Providers in Oregon is hiring a Hospice RN for an 0.8 FTE night shift position. The RN will deliver professional nursing services to hospice patients per the plan of care, perform initial and ongoing assessments, and coordinate care with the interdisciplinary team.
Responsibilities include timely documentation, communication with physicians and community resources, participation in IDT meetings, and ensuring compliance with Medicare conditions of participation.
