Job description
Gentiva's Heartland Hospice is seeking a Registered Nurse Case Manager to deliver compassionate hospice care in patients' homes and affiliated settings. You will assess, plan, implement, and evaluate care with a focus on comfort, dignity, and family support.
You will collaborate with physicians, social workers, chaplains, and other team members to ensure personalized plans of care, training for caregivers, and timely documentation. A safety-sensitive role with on-call responsibilities may apply.
