Job description
Moments Hospice is seeking an experienced Hospice RN Clinical Manager/Team Director to lead an interdisciplinary team and ensure holistic, compliant care for patients and families. You will coordinate with nurses, aides, social workers, chaplains, bereavement coordinators, and volunteers, while embracing autonomy and a collaborative team culture.
This role involves travel on the road, overseeing care coordination, policy implementation, and seamless communication with families and physicians.
