Job description
Moments Hospice is seeking an experienced RN Clinical Manager/Team Director to lead an interdisciplinary team delivering compassionate end-of-life care. You will coordinate nurses, aides, social workers, chaplains, and volunteers to ensure holistic, patient-centered plans and clear communication with families and physicians.
You will oversee scheduling, on-call coverage, and quality metrics while fostering a supportive, collaborative work culture across the organization.
