Job description
Heartland Hospice is seeking a Hospice Care Consultant (HCC) to lead strategic referral development and advocate for patients and families. You will educate healthcare partners about hospice options, identify new referral opportunities, and drive outreach with physicians, hospitals, SNFs, and home health professionals.
You will work independently and with clinical teams, bringing 2+ years of healthcare sales and strong communication to build lasting relationships and achieve admissions goals.
