Job description
MJHS is a large not-for-profit health system in the Greater New York area offering home care, hospice, rehabilitation, and elder care services. This role provides psychosocial support to hospice patients and families across inpatient and community settings, collaborating with an interdisciplinary team to develop care and discharge plans.
