Job description
MJHS is a large not-for-profit health system in the Greater New York area seeking a Social Worker to provide psychosocial support to Hospice patients and families across inpatient, home care and nursing home settings. You will join the hospice IDG, coordinating care and discharge planning with clinical teams to ease the burden of life-limiting illness.
Required: MSW from an accredited school, NYS licensure, and at least one year of healthcare social work.
