Job description
Gentiva, part of Heartland Hospice, is seeking a Hospice Social Worker (MSW/LSW) in New Jersey to join our interdisciplinary team and guide patients and families through end‑of‑life care. You will assess psychosocial needs, provide counseling, and coordinate resources in a supportive home‑based hospice setting.
Qualified candidates hold an MSW from a CSWE‑accredited program, possess applicable licensure, and have at least 1 year of healthcare or hospice experience.
