Job description
Gentiva’s Heartland Hospice seeks a dedicated Social Worker (MSW/LSW) to join our compassionate interdisciplinary team. You will support patients and families through serious illness and end-of-life decisions, delivering counseling, care planning, and resource guidance in a home-based hospice setting.
Requirements include an MSW from CSWE-accredited program, NJ license and at least 1 year of relevant healthcare or hospice experience.
