Job description
MJHS in the New York area seeks a licensed social worker to join our hospice and palliative care teams. You will provide psychosocial support to patients and families across inpatient and home settings, and participate in IDG discussions to coordinate care.
We value autonomous professionals who can input clinical data, collaborate with an interdisciplinary team, and develop care plans and discharge plans tailored to each patient’s needs.
