Job description
MJHS is a large not-for-profit health system serving the Greater New York area, offering hospice and palliative care, home care, and related services. We seek a Social Worker to provide psychosocial support to hospice patients and families, acting as a liaison within the IDG and coordinating care plans and discharge planning as needed.
