Hebrew SeniorLife seeks a compassionate Hospice Staff RN/Case Manager to provide home-based end-of-life care to older adults. In this 40-hour role, you will manage a caseload of hospice patients, complete holistic assessments, develop and update individualized care plans, and coordinate services with an interdisciplinary team. Responsibilities include pain and symptom management, patient and family education, accurate EHR documentation, and collaboration with physicians, social work, and spiritual care.
