nAs the Home and Community Bases Services Aide, you will provide support, assistance with personal hygiene and household functions for an individual to be able to remain in their own home.

nnnThis is a Full Time position doing Home Visits for Hospice Care.

n

nnSTNA certification in OH required.

n

nnOur Business Hours are Mon-Fri, 8AM-4:30PM.

n

nnOur agency covers Summit, Stark, and Cuyahoga counties. Candidates must be able to see patients within this area.

n

n nPrimary Responsibilities:

nnnProvide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders

n

nnSupport household tasks, meal preparation, and accompany clients to appointments or errands as needed

n

nnMonitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system

n

nnEnsure a safe environment, operate medical equipment properly, and respond promptly to client needs

n

nnAbility to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

n

nRequired Qualification:

nnCurrent driver's license, vehicle insurance, and reliable transportation or access to public transportation

n nState-Specific Requirements:

nCategory I and II- Home Health Aide- Medicare Certified Home Health Provider

nnOH: No Additional Requirements.

n nCategory III and IV:

nnOH: Must have successfully completed an approved Ohio Department of Health State Tested Nurse Aide (STNA) training program and competency, OR meet the qualifications to be Medicare Certified Home Health Aide, OR have at least one (1) year supervised employment experience as a Home Health Aide or Nurse Aide and has successfully completed a competency evaluation, OR has successfully completed a Council on Aging Learning Advantages (COALA) Home Health training program, a certified vocational or other sixty (60) hour training and competency evaluation program that meets the requirements listed in rule 173-39- 02.11(C)(3)(a)(v)(b).

n nPreferred Qualifications:

nnnCurrent CPR certification or ability to complete within 90 days of hire

n

nn6+ months of home care experience

n

nnAbility to work flexible hours and independently

n

n nPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $14.00 to $25.10 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
