Job description
Residential Home Health and Hospice is seeking a Hospice Transitional Nurse Liaison to partner with field staff and leaders in delivering exceptional hospice support. The role focuses on comprehensive patient assessments, care coordination, and education to ensure a smooth transition of care.
The position requires an ADN, RN license, and at least two years of nursing experience with hospice preferred. Proficiency with EPIC, HCHB, and MS Office is desirable.
