Job description
St. Croix Hospice is seeking a Hospice Transition Nurse (RN) in St. Peters, MO. The role focuses on educating hospitals, nursing homes, and community partners about hospice programs and services, while coordinating patient transitions and admissions.
The nurse acts as a resource to hospitals, navigates EMR systems, and conducts goals-of-care meetings with patients and families. Strong emphasis on building referral networks and mentoring newer RNs.
