Job description
Transitions Care is seeking a compassionate Registered Nurse to provide hospice visit care in patients’ homes and facilities. In this role, you’ll deliver skilled nursing assessments, manage symptoms, administer medications, and implement individualized care plans focused on comfort and quality of life. You will educate and support patients and families, coordinate closely with an interdisciplinary hospice team, and ensure timely documentation. Ideal candidates demonstrate strong clinical judgment, hospice or home health experience, excellent communication, and a deep commitment to dignity, respect, and holistic end-of-life care.
