Job description
NewYork-Presbyterian Hospital is seeking an Advanced Clinical Provider (Physician Assistant) to join the Hospital at Home Program in New York, NY. You will deliver inpatient-level care at the patient's home through daily in-home visits, telehealth, and 24/7 monitoring, collaborating with Medical Directors and care teams.
The role emphasizes complex clinical decision-making, enrollment and discharge management, and coordination across EDs, inpatient units, and the Virtual Unit.
