Job description
Northwestern Medicine is seeking a motivated Physician Assistant or Acute Care Nurse Practitioner to join our Hospital at Home team in Lake Forest, Illinois. This role is focused on inpatient care, providing clinical expertise through direct patient interactions and care, while ensuring a smooth transition from hospital to home.
