Job description
Lee Health is seeking a Hospitalist at Home Physician to deliver acute-level care in patients’ homes through a hybrid model that combines in-home visits, virtual rounding, and remote monitoring. This role leads care delivery under a value-based, patient-centered framework.
Ideal candidates are board-certified/eligible in Internal or Family Medicine with hospital medicine experience and a passion for innovation and technology-enabled care.
