nThe Hospital Care Coordinator-RN Case Management responsibilities include:
nnnProvides linkage, monitoring, planning and advocacy for our population to achieve and maintain maximum functioning.
n
nnPossesses a clear understanding of the available resources to promote quality cost effective outcomes.
n
nnEffectively maintains and manages assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served.
n
nnCoordinates discharge planning efforts through assessment, referral, and interdisciplinary planning.
n
nnAbility to adjust to changing priorities in a fast-paced environment.
n
n nPosition details:
nnnLocation: Winter Haven Hospital - Winter Haven, FL
n
nnStatus: Full time, 40 hours per week. Exempt.
n
nnSchedule: Monday - Friday 8:30 AM - 5:00 PM
n
nnWeekend Requirement: Occasional
n
nnOn Call: No
n
n nCertifications and Licensures:
nnnRequired RN (Registered Nurse)
n
nnPreferred ACM (Case Management)
n
nnPreferred CCM (Case Manager)
n
n nEducation:
nnnRequired Associates of Nursing or
n
nnPreferred Bachelor of Nursing
n
n nExperience:
nnnRequired 2 years in Acute Care
n
nnRequired 2 years Registered Nurse
n
