Job description
Ochsner Health System is seeking a Registered Nurse Case Manager for the Shreveport area. The role involves critical responsibilities in managing patient care throughout hospitalization, ensuring medically necessary care while collaborating with a multi-disciplinary team and post-acute providers.
The ideal candidate will have a diploma or degree in nursing, 3 years of hospital experience, and must hold a valid Louisiana RN license. Experience in case management or discharge planning is preferred.
