Job description
Duke University Health System in Durham, NC, is seeking a Case Manager to coordinate discharge planning across the continuum of care. You will assess patient needs, develop individualized discharge plans, and collaborate with care teams to ensure safe transitions and optimal outcomes.
The ideal candidate holds a BSN or MSW, has at least 3 years of experience, and will obtain a Case Management Certification within 3 years. RN licensure in NC is required for BSN holders.
