Job description
The Liaison position supports hospital discharge by coordinating O2 portability, nebulizers, and DME with case managers and facility staff. You will educate on insurance criteria, equipment needs, and programs available to facilitate discharge.
Daily duties include rounds to case managers, coordinating with centers for home setup, and completing setup paperwork for equipment. Familiarity with CMNs and documentation for reimbursement is beneficial.
